Electronic Consent
Publication 1179 of the Internal Revenue Service requires that an Issuer receive affirmative consent from recipients in order to deliver tax documents electronically. The following will provide you with the disclosures required under IRS requirements:
- If you do not consent to receive your documents electronically, a paper copy will be provided.
- Your consent is valid for all document types, including, but not limited to, Forms 1095-B, 1095-C, 1099-MISC, 1099-NEC, 1099-DIV, and 1099-INT, and is active immediately following the date of the consent and remains active indefinitely unless you explicitly opt-out of receiving your forms electronically (see #4 for opt-out instructions).

Requesting a paper copy of your documents will not be treated as a withdrawal of consent.
To obtain a paper copy, please contact your account administrator. - The consent may be withdrawn at any time by furnishing the withdrawal in writing (electronically or on paper) to your account administrator. Confirmation of the withdrawal will also be in writing (electronically or on paper). A withdrawal of consent does not apply to documents that were sent to you in accordance with IRS requirements before the effective date of the withdrawal of consent.
- Your account administrator will cease to provide you with documents if you provide a notice to withdraw consent, if you are no longer a recipient, or if regulations change to prohibit the form of delivery. Your documents will be available until the first business day after October 15th at least 1 year from the date they were issued.

The e-delivery website is designed to support Internet Explorer 11+, Google Chrome, Mozilla Firefox, Microsoft Edge, and many other standards compliant browsers.
PDF content has been tested and verified to work on Adobe Acrobat Reader.
